What you’ll need to apply

Please have the following documents ready to complete your application.

1. 407 ETR Privacy Consent Form Please download and fill out the 407 ETR Privacy Consent Form

2. Proof of residency

   • Utility bill

   • Rental agreement

   • Mortgage documents

3. Proof of plate ownership

   • Plate permit

4. Proof of income Proof of income must be provided for all individuals over the age of 18 in the household

   • Notice of Assessment for most recent tax year

   • In the absence most recent tax year Notice of Assessment the following may be submitted:

     • Last two pay stubs

     • Statements of remuneration from federal and/or provincial income assistance programs

   • The application review period may be extended if the proof of income is not the most recent tax year Notice of Assessment
